Nursing Care for a stroke patient at home

Abstract

Cerebrovascular disease (Stroke) is a common neurological disease. The cause may be a ruptured blood vessel or blockage from a blood clot causing damage to brain tissue. Currently, there is a tendency to increase and is a major public health problem in Thailand. It is leading cause of death. The surviving patients are often left with disabilities, discouragement, which affects the family in the long term. This report presents a case study of nursing care for a stroke patient at home. It aims to illustrate the work of community nurses on patients and their families which is considered a very important role to set a discharge planning with the team while the patient was in the hospital, prepare the patient and family and co-ordinate with network staff of sub-district health promoting hospitals and village health volunteers to give nursing care for a stroke patient at home. Patient care uses nursing theory concepts and various fields of knowledge to be appropriately applied to provide holistic nursing care and empowering the family. Outcome showed that patients were quickly recover and had better self-care. There were no complications, As a result, patients and their families have a good quality of life.
