บทคัดย่อ

การวิจัยเชิงวิเคราะห์แบบภาคตัดขวางนี้มีวัตถุประสงค์เพื่อศึกษาระดับ ความสัมพันธ์ และอิทธิพลของภาวะเปราะบาง ภาวะซึมเศร้า และสมรรถนะร่างกายต่อคุณภาพชีวิตของผู้สูงอายุที่เป็นโรคเบาหวาน จากการสุ่มกลุ่มตัวอย่างอย่างง่าย จำนวน 258 คน เก็บรวบรวมโดยใช้แบบสอบถามระหว่างวันที่ 1 เมษายน 2565 – 31 มกราคม 2566 ในพื้นที่จังหวัดแม่ฮ่องสอน และตรวจสอบคุณภาพเครื่องมือ ข้อมูลทั่วไป (IOC=1.00) แบบประเมินภาวะซึมเศร้า (IOC=1.00 , Cohen's Kappa= 0.80) แบบประเมินภาวะเปราะบาง (IOC=1.00 , 𝛼 =0.81) แบบทดสอบสมรรถนะร่างกาย (IOC=0.67 , ICC = 0.92 ) และแบบประเมินคุณภาพชีวิต (IOC= 1.00, 𝛼 =0.89) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา สหสัมพันธ์ของเพียร์สัน และการวิเคราะห์ถดถอยพหุคูณ ผลการศึกษาพบว่า กลุ่มตัวอย่างส่วนใหญ่อยู่ในภาวะเปราะบาง (กลุ่มภาวะเสี่ยงสูง มีปัญหาสุขภาพสูง ร้อยละ 65.1 มีระดับภาวะซึมเศร้า (เสี่ยงสุขภาพจิตปานกลาง) ร้อยละ 45.0 และคุณภาพชีวิตมีความสัมพันธ์ทางบวกกับสมรรถนะร่างกาย (r = 0.58, p < 0.01) และมีความสัมพันธ์ทางลบกับภาวะซึมเศร้า (r = -0.71, p < 0.01) และภาวะเปรา-ะบาง (r = -0.65, p < 0.01) อย่างมีนัยสำคัญทางสถิติ ผลการวิเคราะห์พบว่า ภาวะซึมเศร้า ภาวะเปราะบาง และสมรรถนะร่างกาย สามารถร่วมกันทำนายคุณภาพชีวิตได้ร้อยละ 64 (R² = 0.64) โดยภาวะซึมเศร้าเป็นตัวแปรที่มีอิทธิพลทางลบต่อคุณภาพชีวิตมากที่สุด (β = -0.55) สรุปได้ว่า ภาวะซึมเศร้าเป็นปัจจัยสำคัญที่สุดที่ส่งผลต่อคุณภาพชีวิตของผู้สูงอายุที่เป็นโรคเบาหวาน รองลงมาภาวะเปราะบาง และสมรรถนะร่างกาย ดังนั้น จึงควรมีการพัฒนารูปแบบการดูแลที่บูรณาการการคัดกรองและจัดการภาวะซึมเศร้าควบคู่ไปกับการฟื้นฟูสมรรถนะร่างกายเพื่อยกระดับคุณภาพชีวิตที่ดีต่อไป
