Development of a telemedicine service system for working-age patients with non-communicable diseases

Abstract
Telemedicine has been introduced into health service systems as a way to improve patient-centered care. This research and development study aimed to: (1) analyze the situation and needs regarding a telemedicine system, (2) develop a telemedicine system, (3) compare service waiting times, and (4) evaluate patient satisfaction among working-age people with non-communicable diseases (NCDs) using the telemedicine system at the Chalerm Phra Kiat 60th Anniversary Queen's Health Center, Khlong Masong, Nonthaburi Province. The sample group consisted of 58 participants: 13 people for in-depth interviews (6 patients and 7 health staff), selected by purposive sampling, and 45 people for the telemedicine system group, with the sample size determined using Cohen's method through G*Power at a power level (1-β err prob) of 0.95. The researcher used the PARI Model, which consists of four stages: Prepare, Agile design and development cycle, Refine, and Implement. Both qualitative and quantitative data were analyzed, and service times were compared using a paired t-test. The findings showed that: (1) patients needed a telemedicine system that was easy to use, convenient, and allowed them to see a doctor at their scheduled appointment time; (2) the developed system matched the users' needs; (3) the service time through the telemedicine system was significantly shorter than the traditional method (p < 0.05); and (4) overall satisfaction was at the highest level (mean = 4.83, S.D. = 0.28). Introducing telemedicine as an additional channel in primary health care is an important approach to reducing inequality in access to health services and ensuring that care reaches all patients in a way that truly meets their needs.
