ผลการผ่าตัด Supracondylar humeral fractures ในเด็ก โดยวิธี Anterior approach

บทคัดย่อ
ภูมิหลัง: การผ่าตัดจัดเรียงกระดูกและยึดตรึงในผู้ป่วยเด็กซึ่งมีกระดูกหักชนิค supracondylar humeral fractures มีหลายเทคนิค ซึ่งมีผลการรักษาและภาวะแทรกซ้อนที่แตกต่างกัน วัตถุประสงค์: เพื่อศึกษาผลการรักษาผู้ป่วย supracondylar humeral fractures แบบ ไม่มีแผลเปิด Gartland type III ด้วยการผ่าตัด โดยวิธี anterior approach เปรียบเทียบกับการผ่าตัดด้วยวิธีอื่น ๆ ผู้ป่วยและวิธีการ: รวบรวมผู้ป่วย supracondylar humeral fractures แบบไม่มีแผลเปิด Gartland type III ซึ่งได้รับการผ่าตัดที่โรงพยาบาลสกลนคร ระหว่างเดือนมกราคม 2543 ถึงเดือนมิถุนายน 2547 และติดตามผลการรักษานาน 6 เดือน ผลการศึกษา: จากการศึกษาพบผู้ป่วย supracondylar humeral fractures ทั้งหมด 123 ราย ได้รับการผ่าตัดด้วยวิธี anterior approach 52 ราย (ร้อยละ 42.2) และผ่าตัดด้วยวิธีอื่น ๆ 71 ราย (ร้อยละ 57.8) อายุเฉลี่ยในกลุ่ม anterior approach 6.90±2.15 ปี กลุ่มวิธีอื่น 7.27±2.78 ปี ข้อมูลทั้งด้านเพศ และแขนข้าง ที่หักของทั้ง 2 กลุ่มไม่มีความเตกต่างกัน ระยะเวลาในการผ่าตัดของกลุ่ม anterior approch จะสั้นกว่าในกลุ่มวิธีอื่น ๆ อย่างมีนัยสำคัญทางสถิติ ผลการรักษาอื่น ๆ ได้แก่ Baummann's angle, จำนวนองศาของ motion loss, จำนวนองศาของ carrying loss ไม่มีความแตกต่างกัน นอกจากนี้ภาวะแทรกซ้อนที่สำคัญพบน้อยเพียงร้อยละ 1.9 สรุป: การผ่าตัดรักษา supracondylar humeral fractures โดยวิธี anterior approach เป็นวิธีที่ดีและมีประสิทธิภาพวิธีหนึ่งซึ่งทำได้ง่าย, ใช้เวลาในการผ่าตัดสั้นกว่าและภาวะเทรกซ้อนต่ำกว่าผ่าตัดวิธีอื่น ๆ
