A Development of Family-Centered Care Model for Older Adults with Dementia

Objectives: To develop a community-based, family-centered care model for older adults with dementia.
Methods: This research and development study was conducted in two phases from April to August 2025. Phase 1 involved a situation analysis, while phase 2 focused on model development and outcome evaluation. Participants included family caregivers of older adults who screened positive for cognitive impairment, healthcare providers, and village health volunteers. Research tools consisted of semi-structured interviews, demographic questionnaires, a dementia care knowledge test, and a caregiver satisfaction questionnaire. Qualitative data were analyzed using content analysis, while quantitative data were analyzed using descriptive statistics and paired t-tests.
Results: The situation analysis revealed five key themes: memory and cognitive problems, roles and approaches of families and healthcare teams, supportive factors in caregiving, barriers and challenges in family-involved care, and recommendations for community-based dementia care. The developed model comprised three activities: dementia awareness and literacy, group experience-sharing, and experiential learning activities. The model’s outcomes showed that caregivers’ mean knowledge scores after the intervention were significantly higher than before (13.11±1.45 vs.9.80±2.83; p<0.001). Moreover, 73.33% of caregivers reported the highest level of satisfaction.
Conclusions: The developed model enhanced caregiver knowledge and satisfaction by addressing specific needs, building confidence, and providing psychosocial support. This model can be applied to care for older adults with dementia.
