Effect of Intravenous Iron Sucrose in Heart Failure Patients with Iron Deficiency: A Randomized Controlled Trial

Abstract

Objectives: To evaluate the effects of IV Iron Sucrose on iron status, left ventricular ejection fraction (LVEF), and clinical outcomes in patients with heart failure.

Methods: This randomized, open-label, outcome assessor-blinded controlled trial included 60 outpatients with heart failure with reduced ejection fraction (HFrEF) and iron deficiency. Participants were randomized into two groups (n=30 per group): the intervention group received IV Iron Sucrose 200 mg, and the control group received standard care. Over a 24-week follow-up period, the primary outcomes measured were LVEF, iron status (ferritin and TSAT), and hemoglobin levels. Secondary outcomes included NYHA functional class, quality of life (MLHFQ score), and HF-related hospitalization rates. Data were analyzed using independent t-test, Mann-Whitney U test, and chi-square test.

Results: At 24 weeks, the Iron Sucrose group demonstrated significantly greater improvements compared to the control group: LVEF increased [Median (IQR) = 36.55% (29.50–41.00) vs. 30.50% (25.00–35.00); p=0.002], ferritin levels increased [251.50 ng/mL (221.00–298.40) vs. 68.05 ng/mL (54.00–76.00); p< 0.001], TSAT increased [25.75% (20.00–29.00) vs. 18.00% (16.00–20.00); p=0.002], and hemoglobin levels increased [14.00 g/dL (13.10–15.00) vs. 12.95 g/dL (12.00–13.80); p=0.001]. The treatment group also showed improved NYHA functional class (p<0.001), better quality of life(MLHFQ) (p=0.001), and reduced hospitalization rates (p=0.007).

Conclusions: IV Iron Sucrose significantly improves cardiac function, quality of life, and reduces hospital readmissions.
