The effect of the case management on health behaviors of essential hypertensive patients at Banpong Hospital

Abstract

The purpose of this research was to study the effect of the case management in the essential hypertensive patients on Health behavior, blood pressure level and body mass index (BMI). The samples were 30 hypertension patients, have blood pressure level greater than or equal to 160/100 mmHg attending at medical outpatient department in Banpong hospital from December 2023 to April 2024. The research instrument was case management, which consisted of: 1) health assessment, health education by nurse and multidisciplinary teams, referral to sanitarians for continuity of care, and following up patient visits by telephone and 2) health behavior interview form. The content verification by 3 experts, the accuracy was determined by the Cronbach coefficient was .79. Data were analyzed by percentages, mean, standard deviation and paired t-test.

The results showed that a statistically significant improvement in better health behavior after the intervention ( =3.29 SD=0.42). The level of systolic and diastolic blood pressure level in post–investigation was significantly lower than pre–investigation. (p < 0.05) However, the results on BMI, showed no significant differences in each group (p > 0.05).
