บทคัดย่อ

วัตถุประสงค์: การวิจัยเชิงวิเคราะห์ความสัมพันธ์เชิงทำนายแบบย้อนหลัง (Retrospective Predictive Correlational Research) นี้ มีวัตถุประสงค์เพื่อวิเคราะห์ปัจจัยทำนายภาวะพึ่งพิงด้านกิจวัตรประจำวันของผู้สูงอายุ ในเขตรับผิดชอบของโรงพยาบาลส่งเสริมสุขภาพตำบลชีน้ำร้าย 2 จังหวัดสิงห์บุรี ซึ่งเป็นพื้นที่ที่มีบริบทสังคมผู้สูงอายุระดับสุดยอด (Super-Aged Society)

วิธีศึกษา: กลุ่มตัวอย่างคือประชากรผู้สูงอายุที่มีอายุ 60 ปีขึ้นไป จำนวน 500 ราย คัดเลือกแบบเจาะจง (Purposive sampling) จากฐานข้อมูลระบบอิเล็กทรอนิกส์ (HOSxP_PCU) และเวชระเบียนประจำปีงบประมาณ พ.ศ. 2567 เครื่องมือที่ใช้ในการรวบรวมข้อมูลประกอบด้วย ข้อมูลด้านประชากรศาสตร์ แบบประเมินความสามารถในการปฏิบัติกิจวัตรประจำวัน (Barthel ADL Index) แบบประเมินภาวะถดถอย 9 ด้าน และแบบคัดกรองภาวะซึมเศร้า (2Q/9Q) วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา และวิเคราะห์ปัจจัยทำนายด้วยสถิติการถดถอยโลจิสติกแบบทวิ (Binary Logistic Regression)

ผลการศึกษา: กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง ร้อยละ 62.4 มีอายุเฉลี่ย 72.8 ปี (S.D. = 7.4) และพบความชุกของกลุ่มที่เกิดภาวะพึ่งพิงด้านกิจวัตรประจำวัน (คะแนน ADL < 11) ร้อยละ 15.8 ผลการวิเคราะห์สมการถดถอยโลจิสติกแบบทวิ พบว่ามี 2 ปัจจัยที่สามารถทำนายการเกิดภาวะพึ่งพิงได้อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 ได้แก่ ประวัติการหกล้มใน 6 เดือนที่ผ่านมา (OR = 3.68, 95% CI: 1.82-7.45, p < .001) และ ภาวะซึมเศร้า (OR = 2.31, 95% CI: 1.20-4.45, p = .015) สำหรับปัจจัยด้านอายุ จำนวนโรคประจำตัว และปัญหาการเคลื่อนไหว ไม่พบความสัมพันธ์เชิงทำนายอย่างมีนัยสำคัญทางสถิติในสมการนี้

สรุป: ประวัติการหกล้มและภาวะซึมเศร้า เป็นปัจจัยสำคัญที่ทำให้ผู้สูงอายุเข้าสู่ภาวะพึ่งพิง ข้อค้นพบนี้สะท้อนให้เห็นถึงภาวะพึ่งพิงเป็นสิ่งที่สามารถชะลอได้หากมีการจัดการปัจจัยเสี่ยงที่ปรับเปลี่ยนได้ ดังนั้น หน่วยบริการปฐมภูมิและองค์การบริหารส่วนตำบล ควรบูรณาการนโยบายและจัดสรรงบประมาณกองทุนการดูแลระยะยาว (LTC Fund) เพื่อมุ่งเน้นการปรับปรุงสภาพแวดล้อมเพื่อป้องกันการหกล้ม และการคัดกรองสุขภาพจิตเชิงรุก เพื่อรักษาความสามารถในการพึ่งพาตนเองของผู้สูงอายุในชุมชนให้ยาวนานที่สุด
