บทคัดย่อ
หลักการและเหตุผล: การบาดเจ็บที่ศีรษะไม่รุนแรง (Mild Traumatic Brain Injury: MTBI) เป็นภาวะฉุกเฉินที่พบบ่อยในแผนกฉุกเฉินของโรงพยาบาลชุมชน การตรวจเอกซเรย์คอมพิวเตอร์สมอง (CT Brain) มีความสำคัญในการตรวจพบภาวะเลือดออกในสมองที่ต้องการการรักษาอย่างเร่งด่วน อย่างไรก็ตาม การส่งตรวจโดยไม่มีการคัดกรองที่เหมาะสมก่อให้เกิดภาระค่าใช้จ่ายและการสัมผัสรังสีโดยไม่จำเป็น
วัตถุประสงค์: เพื่อศึกษาความชุกและประเภทของความผิดปกติที่พบจากการตรวจ CT Brain และระบุปัจจัยที่มีความสัมพันธ์กับการพบความผิดปกติดังกล่าวในผู้ป่วยบาดเจ็บที่ศีรษะไม่รุนแรง (Mild Traumatic Brain Injury: MTBI) ที่โรงพยาบาลสมเด็จพระยุพราชกระนวน จังหวัดขอนแก่น
วิธีการศึกษา: การวิจัยเชิงวิเคราะห์แบบย้อนหลัง (Retrospective Analytical Study) ศึกษาจากเวชระเบียนผู้ป่วยที่ได้รับการวินิจฉัยว่ามีการบาดเจ็บที่ศีรษะไม่รุนแรง (Glasgow Coma Scale 13-15) อายุตั้งแต่ 15 ปีขึ้นไป และได้รับการตรวจ CT Brain ที่โรงพยาบาลสมเด็จพระยุพราชกระนวน ระหว่างวันที่ 1 มกราคม 2565 ถึง 31 ธันวาคม 2567 รวบรวมข้อมูลตัวแปรอิสระ ได้แก่ เพศ อายุ กลไกการบาดเจ็บ อาการและอาการแสดงทางคลินิก โรคประจำตัว และประวัติการใช้ยา วิเคราะห์ข้อมูลด้วยสถิติพรรณนา Chi-square test, Fisher's exact test และ Multivariate logistic regression พร้อมประเมินประสิทธิภาพของโมเดลด้วยค่าพื้นที่ใต้โค้ง ROC และ Hosmer-Lemeshow goodness-of-fit test กำหนดระดับนัยสำคัญทางสถิติที่ p < 0.05
ผลการศึกษา: ผู้ป่วยที่เข้าเกณฑ์การศึกษาทั้งหมด 440 ราย เป็นเพศชายร้อยละ 58.6 พบความผิดปกติจากการตรวจ CT Brain 81 ราย คิดเป็นร้อยละ 18.4 (95%CI: 14.8–22.0) ความผิดปกติที่พบบ่อยที่สุดคือ เลือดออกใต้เยื่อหุ้มสมองชั้นอะแร็กนอยด์ (ร้อยละ 65.4) รองลงมาคือเลือดออกใต้เยื่อหุ้มสมองชั้นดูร่า (ร้อยละ 60.5) เลือดออกในเนื้อสมอง (ร้อยละ 35.8) และเลือดออกเหนือเยื่อหุ้มสมองชั้นดูร่า (ร้อยละ 27.2) โดยผู้ป่วยร้อยละ 56.8 พบความผิดปกติมากกว่า 1 ชนิดร่วมกัน ปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติต่อการพบความผิดปกติใน CT Brain ในการวิเคราะห์หลายตัวแปร ได้แก่ อุบัติเหตุรถจักรยานยนต์ (Adjusted OR = 2.15, 95%CI = 1.27–3.65, p = 0.004) และอายุน้อยกว่า 60 ปี (Adjusted OR = 1.78, 95%CI = 1.06–3.00, p = 0.030) โดยโมเดลมีความสามารถในการจำแนกผู้ป่วยกลุ่มเสี่ยงในระดับปานกลาง (AUC-ROC = 0.673, 95%CI: 0.604–0.740) ด้านผลลัพธ์ทางคลินิก ไม่พบการผ่าตัดทางระบบประสาท การส่งต่อ หรือการเสียชีวิต และผู้ป่วยร้อยละ 99.5 มีคะแนน Glasgow Outcome Scale = 5 ที่ 1 เดือน
สรุป: อุบัติเหตุรถจักรยานยนต์และอายุน้อยกว่า 60 ปีเป็นปัจจัยที่มีความสัมพันธ์อิสระต่อการพบความผิดปกติใน CT Brain ในผู้ป่วย MTBI ที่โรงพยาบาลชุมชน แม้โมเดลจะมีความสามารถในการจำแนกผู้ป่วยกลุ่มเสี่ยงอยู่ในระดับปานกลาง (AUC-ROC = 0.673) ปัจจัยที่ระบุได้ในการศึกษานี้อาจเป็นข้อมูลเบื้องต้นที่มีประโยชน์ต่อการพัฒนาแนวทางการตัดสินใจส่งตรวจ CT Brain ที่เหมาะสมกับบริบทของโรงพยาบาลชุมชน ซึ่งควรได้รับการตรวจสอบความถูกต้องไปข้างหน้า (prospective validation) ก่อนนำไปใช้จริงในเวชปฏิบัติ เพื่อช่วยลดการส่งตรวจที่ไม่จำเป็นและลดความเสี่ยงจากการได้รับรังสีโดยไม่มีข้อบ่งชี้
