Development of Self–care Management Promotion Model for Heart Failure Patients at the Heart failure Clinic in Yasothon Hospital

Abstract

            This research and development aimed to develop Self–care management promotion model for heart failure patients at the heart failure clinic in Yasothon Hospital and evaluate the effect after development. The sample consisted of 45 patients with heart failure who were served in the heart failure clinic, a cariologist, a nurse with experience in heart disease care, 2 pharmacists, 2 nutritionists and 2 physical therapists. The research phase was divided into 3 phases: phase 1; analysis and synthesis of prototype, phase 2; prototype development and phase 3; test the effectiveness of new knowledge prototype. Data were collected from May, 2026 to January 2027. The research instrument included self-care management promotion model, manual of self–care management of patients with heart failure, data collection form, focus group form, self-care of heart failure questionnaire by Riegel et al., (2004) which was translate into Thai by Suwanno, et al., (2008), quality of life of heart failure questionnaire and the six minute walk test (6-MWT) recorder. Data were analyzed by mean, standard deviation and T test.

            The result of this study revealed that the self-care management quality of life and 6-MWT among patients with heart failure after receiving self-care management promotion model were significantly better than before using this service p<.05 level. Non of the sample readmission within 6 months.

            The result of this study indicated promote to implement in to the heart failure clinic in the others hospital, to help them effectively self-care management their heath, attain a better quality of life and reduces readmission rate.
