บทคัดย่อ

ความเป็นมา: ภาวะหายใจลำบากในผู้ป่วยเด็กพบได้บ่อย ปัจจุบันมีการนำ heated humidified high flow nasal cannula (HHFNC) มาใช้มากขึ้น การทราบปัจจัยที่มีผลต่อการรักษาล้มเหลวจะช่วยให้ผู้ดูแลมีการเฝ้าระวังติดตามเพื่อให้เกิดผลลัพธ์การรักษาที่ดี

วัตถุประสงค์: เพื่อศึกษาอัตราความล้มเหลวในการใช้ HHFNC และปัจจัยที่มีความสัมพันธ์กับความล้มเหลวในการใช้ HHFNC ในผู้ป่วยเด็กโรงพยาบาล ๕๐ พรรษา มหาวชิราลงกรณ

วิธีการศึกษา:  เป็นการศึกษาแบบย้อนหลัง กลุ่มตัวอย่างได้จากเวชระเบียนผู้ป่วยเด็กอายุมากกว่า 1 เดือน -15 ปี ที่ได้รับการรักษาด้วย HHFNC ระหว่าง 1 ตุลาคม พ.ศ. 2565 ถึง 30 กันยายน พ.ศ.  2568 วิเคราะห์ข้อมูลทั่วไปและอัตราความล้มเหลวของการใช้ HHFNC ด้วยสถิติเชิงพรรณนา วิเคราะห์ปัจจัยที่มีความสัมพันธ์กับความล้มเหลวในการใช้ HHFNC ด้วยสถิติ Chi-square test และ binary logistic regression analysis

ผลการศึกษา:  ผู้ป่วยที่ทำการศึกษาทั้งหมด 445 ราย มีอายุเฉลี่ย 36 เดือน เป็นเพศชายร้อยละ 61.6 ป่วยเป็นโรคปอดอักเสบ ร้อยละ 72.6 ส่วนใหญ่มีโรคประจำตัวเป็นโรคหอบหืด ร้อยละ 14.3 พบการรักษาด้วย HHFNC ล้มเหลว 30 ราย คิดเป็นอัตราร้อยละ 6.7 ที่ 2 ชั่วโมง ค่า ROX index ของกลุ่มล้มเหลวต่ำกว่ากลุ่มสำเร็จ (5.68 ± 2.16 เทียบกับ 9.26 ± 32.07, p value 0.001) และที่ 4 ชั่วโมง ค่า S/F ratio ของกลุ่มล้มเหลวต่ำกว่ากลุ่มสำเร็จ (200.84 ± 84.88 เทียบกับ 215.94 ± 80.74, p value  0.017) ปัจจัยที่สัมพันธ์กับโอกาสการเกิดความล้มเหลวต่อการรักษาด้วย HHFNC ได้แก่ ได้แก่ ประวัติเคยใช้เครื่องช่วยหายใจ (OR 4.35, 95% CI 1.23-14.29) อัตราการหายใจลดลง < ร้อยละ 20 ภายใน 2 ชั่วโมง (OR 10.78, 95% CI 1.25-92.96) และ Max FiO2 > 60% (OR 12.50, 95% CI 4.55-33.33)

สรุป: พบมีอัตราความล้มเหลวของการใช้ HHFNC ร้อยละ 6.74 ปัจจัยที่สัมพันธ์กับโอกาสการเกิดความล้มเหลวต่อการรักษาด้วย HHFNC ได้แก่ ประวัติการใช้เครื่องช่วยหายใจ  อัตราการหายใจลดลง < ร้อยละ 20 ภายใน 2 ชั่วโมง และ max FiO2 > 60% พบ S/F ratio และ ROX index ในกลุ่มล้มเหลวมีค่าต่ำกว่าแบบมีนัยสำคัญทางสถิติที่ 4 และ 2 ชั่วโมงตามลำดับ
